1. Low Contrast Between the Code and Background
Phone cameras rely on a clear difference between the dark and light parts of the code. A QR code in light gray on a white background, or a color combination with similar brightness, often can't be read even though it looks fine to the human eye.
2. The Code Is Too Small
A QR code needs to be large enough, relative to the scanning distance, for the camera to resolve its individual squares. A code that works fine on a phone screen can become unscannable when printed too small on a flyer meant to be scanned from a few feet away.
3. Missing Quiet Zone
QR codes need a blank margin (the "quiet zone") around all four sides. If the code is placed too close to text, images, or the edge of the page, scanners can fail to detect where the code starts and ends.
4. Blur or Low Print Resolution
Printing a QR code at low resolution, or photographing one that's slightly out of focus, can blur the fine detail scanners depend on — especially for codes carrying a lot of data.
5. Too Much Data Packed Into the Code
QR codes have to shrink each individual square as more data goes in. A code linking to a very long URL becomes denser and harder to scan reliably than one linking to a short, shortened link.
6. Damage or Obstruction
QR codes do have built-in error correction, but it only covers so much. A fold, a sticker, glare, or physical damage across a large enough portion of the code can push it past what the error correction can recover.
7. A Genuinely Corrupted File
Less commonly, the QR code image itself may have been damaged during export, resizing, or format conversion, corrupting the actual data pattern rather than just the visual presentation.
